To request a certificate email certs@zhins.com with your certificate holders full name and address how they want to be listed. Don't forget to add your name and policy number!

What needs to be included in a certificate request?
You will need to include the policy holders full name, company name or policy number. For the certificate holder we will need the full name and address exactly how they want to be listed on the certificate. Please include any special wording that is being request.
